BMS Sports Physical Information

Physical Information (WCS Board Policy 4.301):

Once you have obtained a physical form filled out by a physician, you will need to make copies to give to the coaches on the first day of tryouts.  Please do not give a coach your original physical, they do not keep them after the season.  If your child plays more than one sport, you will need a copy of your physical for each coach. Students will not be allowed to tryout without a physical.

All students need a sports physical form in order to try out for a sport at Brentwood Middle.  Please follow this procedure:

1. Obtain a sports physical form from the office or the BMS website.  Click here for form.

2. Get a physical from your doctor-preferably after June 1st so that it can used all year.

3. Make at least 5 copies of the signed physical form.

4. On the first day of tryouts, the student should give a copy of the physical form to the coach.  Students will need to do this for each sport in which they participate.

5. Do not give the original copy to the coach!
